Intake & Eligibility
If you or someone you love may have a developmental disability and you’re looking for support, you’ve come to the right place. SCBDD serves people of all ages — from infants and toddlers to adults — and we’re here to help you understand what services might be available and how to get started.
The process begins with a referral. From there, our team will guide you through an evaluation, help determine eligibility, and work with you to build a plan that reflects your goals and needs.
Who Is Eligible?
To receive services through SCBDD, an individual must:
- Have a qualifying developmental disability, which may include intellectual disability, autism spectrum disorder, cerebral palsy, Down syndrome, or other conditions that meet Ohio’s eligibility criteria
- Have experienced the disability before age 22
- Be a resident of Sandusky County, Ohio
- Have a condition that substantially limits major life activities
Eligibility is determined through a formal evaluation process, not assumed based on diagnosis alone. Our team will walk you through the evaluation when the time comes.
How to Get Started
Step 1
Submit a Referral
Fill out the referral form below, or call our Intake Specialist to be referred by phone or through a school, hospital, or healthcare provider.
Step 2
Initial Contact
Our Intake Specialist will contact you to gather more information, answer your questions, and explain the next steps.
Step 3
Evaluation
We’ll schedule an evaluation to gather information about the individual’s strengths, needs, and functional abilities.
Step 4
Eligibility Determination
Based on the evaluation results, SCBDD will determine whether the individual meets eligibility criteria for services.
Step 5
Welcome to SCBDD
If eligible, you’ll be connected with a Service and Support Administrator (SSA) who will work with you to develop a plan and connect you to the right services.

What to Expect
Starting the process can feel overwhelming, especially if this is new for your family.
Here are some things to know:
- There is no cost to apply for or be evaluated for services.
- You do not need to have all the answers before reaching out. We will help you figure out next steps together.
- Evaluation timelines vary, but our team works to complete them as efficiently as possible.
- Being found eligible doesn’t mean you are required to use services — it simply opens the door.
